Pregnancy, delivery, postpartum and newborn is a physiological process, but there is possibility of complications. Puskesmas Gajah II is one of the health centers with hospitalization PONED in Demak district. Based on data from Puskesmas Gajah II, may not find the case of MMR and IMR in 2014. However, in 2015 there were 1 MMR cases of tuberculosis and one case of IMR due to asphyxia. It needs a comprehensive midwifery care by skilled health care personel.
Writing the final report is in the form of case studies with the aim of implementation of midwifery care can be comprehensive which covers pregnancy, delivery, post partum, neonatus and family care plan using obstetric management and documentation SOAP.
The research results obtained by G2P1A0 39 weeks gestasion, pregnant with physiological, spontaneous delivery without complications and no rips perinium, the baby born in a normal state, postnatal physiological. In pregnancy antenatal care is given not meet the standars 10 T, because Ny. S given not TT immunization at this pregnancy. In the second stage of labor gaps obtained is doing infussion although there is no indication and not wearing complete self protection equipment. In the neonatus found a gap in the management of the IMD is performed for 10 minutes. As for visit postpartum and neonatus did not find the gaps. Family care plan given at 6 weeks post partum. She decided to wear a three-month injections at day 44 post partum.
The conclusion from this research is the application of midwifery care there is a gap between theory and practice of midwifery care in the land. The gaps are in the process of midwifery care. Suggestion: Ny. S are able to apply information related to education which has been given, midwife and health care is expected to able to give education comprehensively, institution is able to be references and documentation, while for writer in improving this research results.
