Analisis Kuantitatif Kelengkapan Formulir Resume Medis Pasien Rawat Inap di RSUD RAA Soewondo Pati Triwulan Pertama tahun 2016

Resumes is a summary the maintenance hospital patients made complete and with short accompanied by name and signature physician who tending and can explain important information about patients especially a disease the medical check , and treatment obtained. Incompleteness medical resumes have an impact on the quality of medical record and medical services provided by the hospital leading to the need for control of charging medical resumes with the quantitative analysis. Based on preliminary survey in RSUD RAA Soewondo Pati, from 10 form medical resumes in-patient found 100 % incomplete filling out of form at items signature doctor , 80 % incomplete filling out of form at items date of birth , and 70 % incomplete filling out of form at the diagnosis main items.
Samples were taken by using random sampling with a sample population of 6062 documents and 99 documents. Identification of variables covering the entire contents of the items listed in the medical resume. Instrument research is used a check list table (inclosed).
The results of the study 4 review form medical resumes obtained an identification review of incompleteness highest item is Lounge / class 87%, authentication review of incompleteness, the highest item is name of the doctor 29%, with the criteria of the doctor's name is abbreviated and can’t be read 46%, critical accounting review incompleteness, the highest is item physical examination 76 %, documentation review the highest incompleteness is item unoccupied spaces 94 %. From these statements the authors advise medical records to make a standard number of medical record documents, every month officers make an analysis reports completeness of medical records, held by the socialization of all doctors to complete medical records document after gives medical services.
