The medical record numbering system is not managed properly, there will be duplication of medical record numbers. Duplicate medical record numbers are multiple medical record numbers owned by a patient or one medical record number owned by several patients. The impact if there is duplication of medical record numbering at the Community Health Center is that services are hampered due to the length of time it takes to search for medical record files, the contents of medical records are not continuous, the medical record shelves will quickly fill up, patients who have received new medical record numbers again if they are not known to be duplicate numbers. This research aims to determine what factors can influence the occurrence of duplicate medical record numbers at the Ngaringan Community Health Center, Grobogan Regency.
The type of research used is qualitative research. Data collection methods use observation and interviews. The instruments used were observation checklists and interview guidelines. This research was conducted in May 2023 with the informants being the Head of the Community Health Center and Medical Records Personnel. The results of the research show that the Man Factor is the cause of duplication of medical record numbers at the Ngaringan Community Health Center, Grobogan Regency, due to the education of medical record officers which is not appropriate to their field of work, as well as an insufficient number of medical record personnel and excessive workload. The money factor is the cause of duplication of medical record numbers at the Ngaringan Community Health Center, Grobogan Regency, because there is no budget allocation to improve the quality of education for medical record officers.
