Abstrak
Mutu pelayanan rumah sakit tidak hanya dinilai dari pelayanan dokter, perawat, dan penunjang medis saja, tetapi juga kelengkapan pengisian rekam medis. Setelah penulis melakukan studi pendahuluan, didapatkan data sekunder terkait persentase ketidaklengkapan rekam medis dengan rata-rata ketidaklengkapan sebesar 7,42%. Tujuan penelitian ini adalah untuk mengetahui persentase kelengkapan pengisian rekam medis kasus non-bedah di RSUD Tidar Kota Magelang meliputi review kelengkapan identifikasi pasien, review kelengkapan laporan penting, review kelengkapan autentikasi, dan review kelengkapan teknik pencatatan. Jenis penelitian yang dilakukan adalah deskriptif kuantitatif, dengan menggunakan instrumen penelitian berupa checklist. Teknik pengambilan sampel yang digunakan adalah accidental sampling yang diambil berdasarkan perhitungan sampel menggunakan rumus Slovin. Dari hasil penelitian didapatkan bahwa persentase kelengkapan identifikasi pasien tertinggi terdapat pasa item nomor rekam medis sebesar 60,42% dan persentase terendah terdapat pada item nama pasien, yakni sebesar 50,00%. Review kelengkapan laporan penting didapatkan persentase tertinggi pada formulir Ringkasan Masuk & Keluar, Catatan Perkembangan Pasien Terintegrasi, Hasil Pemeriksaan Laboratorium, dan Ringkasan Pasien Keluar (Resume) dengan persentase 100% dan persentase terendah pada formulir Hasil Pemeriksaan Radiologi sebesar 98,96%. Review kelengkapan autentikasi didapatkan persentase tertinggi terdapat pada item jam pengisian dengan persentase 100% dan persentase terendah sebesar 48,96% pada item tanda tangan. Review kelengkapan teknik pencatatan didapatkan persentase tertinggi pada penggunaan singkatan sebesar 96,88% dan persentase terendah terdapat pada pembetulan yang sesuai, yaitu sebesar 47,92%. Saran yang dapat diberikan adalah sebaiknya label identitas pasien dicetak sesuai dengan kebutuhan, untuk meminimalisir rekam medis tidak memiliki identitas pasien. Bagian IT rumah sakit mensetting agar nama pasien dalam label identitas tidak terpotong, guna menghindari ketidakkonsistenan identitas pasien. Sosialisasi kepada petugas kesehatan yang berhak mengisi rekam medis tentang pengisian dan kelengkapan rekam medis agar sesuai dengan SPO (Standar Prosedur Operasional) yang telah ditetapkan rumah sakit.

Abstract
[English Title : Quantitative Analisys of Non-Surgical Case Medical Rcord in Tidar Hospital of Magelang City] The quality of hospital services is not only assessed by doctors, nurses, and medical support, but also the completeness of medical record filling. After the author conducts preliminary studies, obtained secondary data related to the percentage of incompleteness of medical record with an average of 7.42% incompleteness. The purpose of this research is to know the percentage completeness of medical record filling of non-surgical cases at RSUD Tidar Magelang City includes a review of patient identification completeness, review of important report completeness, review of authentication completeness, and Review of completeness recording techniques. The type of research conducted is a quantitative descriptive, using a research instrument in the form of a checklist. The sampling technique used is a sampling accidental taken based on sample calculations using the Slovin formula. From the results of the study came that the percentage of the highest patient identification equipment there was 60.42% medical record number and the lowest percentage found on the patient's name item, which amounted to 50.00%. A Review of the completeness of the important report obtained the highest percentage on the & Exit summary form, integrated patient development record, laboratory examination result, and a Resume patient summary with a percentage of 100% and The lowest percentage on the radiology test result form of 98.96%. The Review authentication completeness obtained the highest percentage found on the charging hour item with a percentage of 100% and the lowest percentage of 48.96% on the signature item. The Review of completeness of recording techniques obtained the highest percentage of the acronym 96.88% and the lowest percentage found in the corresponding correction, which amounted to 47.92%. The advice that can be given is that the patient should label the identity printed according to the needs, to minimize the medical record has no patient identity. The IT section of the hospital's IT setting so that the patient's name in the identity label is not truncated, in order to avoid inconsistencies in the patient's identity. Socialization to the appropriate health care personnel records a medical record of filling and completeness of the medical record to conform to the SPO (standard operational procedure) that the hospital has set.
