Implementation of medical doctors records records seen from some of the physical aspect seen from the damage to the document caused by the arrangement of space and maintenance of storage places that is temperature, humidity, ventilation, lighting and space security from fire, environmental aspects seen from the prohibition that should not be in langgar and cleanliness of the room, and the chemical aspects in view of the use of contraband ingredients to protect documents from disease vectors. As a result of the destruction of medical record documents so that the resulting data is not accurate. The general purpose of this study is to describe the system maintenance of medical record documents in the outpatient filling room RSUD K.R.M.T Wongsonegoro Semarang. The type of this research is descriptive analytic correlation by using quantitative approach with cross sectional method. The study population is outpatient filling officer in 2017 in RSUD K.R.M.T Wongsonegoro Semarang. Methods of data collection are kuisoner, observation and documentation study. The results showed that 11 respondents stated that maintenance of medical record document on physical aspect with average variable suspension result 43 of this condition included in good enough category, environmental aspect of average percentage of 42 variables this condition included in good category and chemical aspect average the average suspension variable 39 this condition is included in the category of good enough.
