TINJAUAN KELENGKAPAN PENGISIAN FORMULIR INFORMED CONSENT KASUS BEDAHDI RUMAH SAKIT ROEMANI MUHAMMADIYAH SEMARANG

Completeness of informed consent is very important to support the quality of medical records. Writers do after the preliminary survey of 10 informed consent form in Roemani Muhammadiyah Semarang Hospital of four review, four of them have prosentase completeness under 100%. The purpose of this search is to reviewing completeness of informed consent form and causes of incompleteness of the form.
The kind of research is descriptive research. Method of data collection is observation, documentation study and interview. Data were analyzed using descriptive analysis and presented to text and table.
From the research conducted in Roemani Muhammadiyah Semarang Hospital, the result of research shows that the identification of patient are 97% complete and 3% incomplete, important report review are 63% complete and 37% incomplete, authentication review are 78% complete and 22% incomplete, and register review are 80% unavailable and 20% available. Cause of incompleteness informed consent is the doctor not filled out the form according to the responsibility, the doctor forgot to fill out the form, the form is still manual. From the statement, the writers giving advise to socialize the doctors and nurses to complete the informed consent form, improve the completeness of the filling, the nurse check the completeness of filling the doctor in informed consent form.
