Pregnancy, childbirth, postpartum and new born babies is a normal condition, but in the process there are the possibility of a life threatening situation the mother and baby, which resulted in increasing the number of morbidity and mortality of mother and babies. According to the Indonesian Health Ministry statement MMR in Indonesia in 2013 reached 359/100.000 live births and IMR reaches 32/1000 live births.
Writing the final report is in the form of case studies using obstetric management approach uses descriptive research and documentation SOAP conducted in Ny. X in midwives practice independently Rohsiyati, Amd.Keb Sumowono, Semarang regency.
The research results obtained G2P1A0 diagnosis of gestational age 38 weeks pregnant with physiological, spontaneous delivery without complication with tear perinium degree II, the baby in a normal state, during childbirth and physiological family planning.
The conclusion from this research is the application of midwifery care there is a gap between theory and practice of midwifery care in the field. In pregnancy no gaps are not found, the mother suffered only common complaint experienced by mothers in the third trimester, such as frequent urination. At the time of delivery gaps that early initiation of breastfeeding is only done 15 minutes and in labor gaps that not use personal protective equipment are complete. In the post-partum care of mother is not found gaps theory and practice. In the newborn care any gaps timing og giving vitamin K and eye ointments 15 minute after birth and immunization Hb0 given one day after birth Family care plan, mother decided to use birth control implants at day 40 post partum.
Suggestion: midwives can apply the theory to practice in the field as do the IMD for 1 hour, provide vitamin K and eye ointment in the first hour after birth and immunization Hb0 an hour after the baby is born. Puskesmas can provide the infrastructure needed to make midwifery care.
