The WISN analysis of the Radiography Clinical Instructors’needs for the Vocational Higher-Education within Indonesian Teaching Hospitals

Resources of Radiography clinical instructor in the Practice of Radiological services are one of the important elements that are inseparable from the organizations of Radiographic vocational education and Radiology service typically in Indonesian teaching hospitals. The high workload of the clinical instructor of having professional duties and guiding students concerning Diagnostic Radiology services is critical as it affects the clinical instructor performance. The study aims to obtain an overview needs of the Radiography Clinical Instructor and to compare the ratio availability of the Radiography Clinical Instructor within the National Teaching hospital. The study’s design employed a mix-method with a descriptive cross-sectional of collecting data, involving a limited sample that of 30 Clinical Practice Instructors as respondents from 8 Radiology service sites of Indonesian teaching hospitals. Research data were obtained using questionnaires, documentation, supported by collecting data from focus group discussions (FGD) which generated annual service statistics based on the hospital's records. The quantitative analysis with Workload Indicator of Staffing Needs (WISN) was employed to determine the required nominal number of the clinical instructor needs. It was then compared with the actual personnel available to find out the number of the instructors' gap. The work units and categories of Human Resources responsible for organizing clinical guidance activities in Radiology services individually to patients were referred to the structural management of the teaching hospital. The working unit and categories were the Radiology Diagnostic department and Radiographer respectively, corresponding to clinical guidance activities in Radiology services at all of the Radiology sites. The AWT for Clinical Practice Instructors was 2.007,4 the SW shown that every radiology examination requires 19.977,09 patients/year, whereas the AS for Radiography clinical instructor in practice was 0,06. The calculated CPIN shows that each site of the clinical practices at the 8 National Teaching Hospitals (GHCJ, GHHB, GHKS, GHSY, GHSS, GHMS, GHSM, and GHSD) require the nominal procurement of 10 Clinical Instructors (Radiographers). The difference between the nominal procurement and the actual availability of the clinical instructor was 65% or every hospital's needs between 3-4 additional personnel (35%). The rational depiction with WISN shows clearly the nominal numbers of clinical instructor needs by each of the Hospitals. However, the Clinical Practice Instructor shortage certainly does not mean directly disrupt the performance of educational services within the Radiology work organization since this study was still based on self-evaluation internally. The Clinical Practice Instructors’gap is still critical for the planning of the human resource needs in the future. It is suggested that further study to examine the condition of supply and demand shall be performed in the order to match appropriate information about the clinical instructor's needs in amongst the organizations of teaching hospital and the vocational higher education of Radiographers.
