Medical record is the file containing the notes or documents on his identity , examination , treatment , action and other services that have been given to patients . To maintain the quality of service, the hospital especially the medical records of implementing quality monitoring quality medical record file by performing quantitative analysis that aims to determine the quality of service in the installation of medical records through 4 review is a review of identification , review of reporting , review authentication , and a review of records. The purpose of this research is to know the percentage of completeness of the summary forms in and out review patient identification , critical accounting , authentication and registration in Salatiga City Hospital in April-May , 2016.
This type of research is to type a descriptive survey . The population used in this study is a summary form in and out period IV 2015 at Salatiga City Hospital and samples using accidental sampling technique . Data collection method used in this research is the study documentation. The data in this study are typing , classification , entry , saving and tabulation . Analysis of the data in this research is descriptive analisis ( univariate analysis )
The results of the analysis of completeness of the summary forms in and out in hospitals Salatiga on the review of the identification of patients with a complete high of the component names to 78 % , on the review of critical accounting completeness of the highest in the component diagnosis of primary with 96 % , while for a review of authentication there is a 99 % doctor's signature , and on the review of the recording , there were 7 % streaks .
