ABSTRACT
Head and neck cancer is a malignancy in the upper aerodigestive. One method of treatment of head and neck cancer is radiotherapy. Adaptive radiotherapy is a continuous radiotherapy technique that observes anatomic changes to cancer so that it can provide the best radiotherapy results. The procedure of adaptive radiotherapy on head and neck cancer the Radiotherapy Department Lavalette Hospital Malang generally uses the parameters of significantly reducing tumor volume during the radiotherapy period. This study aims to undestand the procedure of adaptive radiotherapy on head and neck cancer, to determine the optimization of adaptive radiotherapy on head and neck cancer.
The type of this study is qualitative research with a case study approach. The study was conducted at the Radiotherapy Department Lavalette Hospital Malang. The methods of data collection is done by observation, interviews and documentation. Data analysis in this study includes data collection, data reduction, data presentation, and conclusions.

The results show that the procedure of adaptive radiotherapy on head and neck cancer consist of the initial consul, CT Simulation, Treatment Planning System (TPS), Specific Quality Assurance (QA), Verification in fractions 1st, 2nd, 3rd followed every 5 fraction, and radation therapy. Consultation with radiation oncology specialist every fifht fraction. Adaptive radiotherapy was carried out when the patient repeated radiation planning which began with re-CT Simulations until radation therapy. Optimization of adaptive radiotherapy in neck head cancer is done by observing changes in tumor volume which can cause a verification shift value of >0,5cm due to improper mask placement. The results of adaptive radiotherapy in neck head cancer can reduce the dose received by OAR
