In RSUD Cilacap there is wrong one problem that is less complete letter demand and permission written from patient related release information medical .
Method The research is descriptive with use case studies for knowing picture release information medical in Cilacap Hospital . Samples taken use formula slovin for calculate the amount of sample to be examined. Presentation of data in form tables , textual and graph .
Results research in Cilacap Hospital there is two Standard Procedure Operations related with release information medical , 99% release information medical in the form of a medical resume for interests claim insurance or bill company , while 1% release information medical in the form of Visum Et repertum for case enforcement the law . The clerk or parts involved composed from Director, Doctor, Nurse, the Implementing Rules of effort and record medical as facilitator . Incompleteness Request letter amounting to 37.64% and incompleteness permission written patient amounting to 41.94% yet held in a manner maximum .
