Pregnancy, childbirth, postpartum and newborns are state of physiological but in the process there is the possibility of the things that can threaten soul mother and baby who can increase the number of morbiditas and mortality mother and baby. Mother mortality rate and Babies mortality rate in Indonesia are still high if compared to the other state of community. According to the Demography Health Indonesian Survey, the number of mother mortality rate in Indonesia of 359/100.000 living birth.
The final report in case studies form by using midwifery management approach 7 steps of Varney and SOAP documentation conducted on Mrs. W age 25 years old in Tolokan Health Polyclinic working area Getasan Primary Health Care Semarang District.
The results of using this study care for Mrs W obtained diagnoses G2P1A0 age pregnancy 39 weeks physiologically, with a complaint weary there is no gaps theory; childbirth physiologically, there are divide the theory that is not done vulva hygiene; the childbed physiological with complaint handling of the pain perineum who got about how to care for the wound perineum and no gaps theory; newborns done orphanage well without any gaps theory; she uses contraception birth control hypodermic 3 months after the childbed and no gaps theory.
The conclusions from this study, there are gap between the orphanage and practice midwifery in the land
